Summer Camp 2026 Registration

11 - 14 December 2026 | INDEMNITY AGREEMENT
By Completing this form and processing payment I hereby confirm that:

1) I am the parent/ legal guardian

2) I give permission for my teen to attend the ChurchAlive Summer Camp December 2026
taking place at Edu Camp Farm / (departing from and returning to ChurchAlive).

3) I hereby indemnify ChurchAlive and Edu Camp Farm, together with their owners,
pastors, employees and volunteers, against any liability arising from any form
of injury, damage or loss howsoever caused. I understand that reasonable care will
be taken to avoid loss, damage or injury, and that I will be contacted as soon as
possible if any incident occurs. In the event of an emergency, I hereby authorise
the leaders appointed by Church Alive to act as guardians on my behalf.

PAYMENT AND CONFIRMATION
